2. Go to‘ Certificate Proformas and Forms’.
3. Click on the ‘Authorization Form New' Option.
4. Download the form in the form of PDF and start filling.

Candidates after filling the above-mentioned details on the form will hand it over to their respective invigilator for their approval.
