• Income and minority declaration-affidavit on non-judicial stamp paper of minimum Rs.10/- or income certificate from the employer (school leaving certificate can also be considered as proof of minority)
• Proof of permanent residence-Attested copy of Domicile Certificate/Ration Card/Electricity bill/Telephone.
