I have filled ugat form and have selected 5 institutes, do i need to fill their form separately too?

No, you are not required to fill out the form separately. If you qualify the cutoff, you will get a call to appear for the interview rounds. If you have a UGAT percentile score that is higher than the required one, colleges will send you a call letter. After that, you will be required to participate in a group discussion and personal interviews at the college to which you have been assigned.
