For me my IISER exam center is in raipur 815 km away from my house and I can't come that far for this exam because of medical reasons of family

If you are unable to attend the exam due to medical or family emergencies, unfortunately, there is no provision for changing the center or taking the exam at a later date. Next time, during the application, select your preferred centers carefully and as early as possible. And, if you have a medical emergency, always obtain a certificate and inform the authorities immediately after the admit card is released.
